I've scanned in the (copy of a copy) OSI PROM Monitor instructions sheets for both the 65V monitor (Challenger I) and 65A monitor (Challenger II and III, OSI-510 CPU board):

It appears that the document has a few skewed pages. unpaper (http://unpaper.berlios.de/) is a nice tool that can correct skewing and more, in case anyone wants to improve their scanned documents.

Neat! I'll rescan in PNM format Monday, and remove the skew. Apparently converting to PNM from PDF is not readily available.

If you run on Linux or unix, you can use gscan2pdf (http://gscan2pdf.sourceforge.net/), which includes unpaper and other tools.
Otherwise, I use pnmtotiff -> tiff2pdf, or pnmtojpeg -> jpeg2pdf (http://www.freshports.org/graphics/jpeg2pdf/).
