It's pretty easy to make one yourself, but if you're lazy like me, you can buy one from B & C ComputerVisions:

Or if you don't already have a luma/chroma video cable, you can buy a ready-made cable with S-Video output:

This lets you use your Commodore's high quality luma/chroma video output with any modern TV with an S-Video input. The picture quality is noticeably sharper and clearer than composite video, and is immensely better than using the RF modulator!

Here's how my C64 looks when connected to a small LCD TV with the S-video adapter. It's only a 640x480 4:3 TV, so the slight graininess you see is mostly caused by the limited resolution of the LCD panel itself, not of the S-Video connection:
