I have only this photo, no more informations.
Never seen this card before, also no informations on minuszerodegrees.net ;)

Questions:

- If it is for ATs, why it have only 8bit ISA slot? Too slow for AT.
- 3 memory banks? My estimate is that there can be 1x 128k piggybacks and 2x 64k

The memory module kits were the chips themselves not the cards, so I suspect somebody may have used the chips in the card that they placed in the box.

Looks kinda BocaRAM card-esque to me

I think I have one of those cards somewhere, same layout and DIP switch bank... came out of an XT computer, not AT.

The card does not look like an IBM product.

Are you sure that card actually came in that box?

Not the card, but the IBM AT "256KB Memory Module Kit" is indeed at minuszerodegrees, in IBM's announcement letter (184-097) for the IBM AT.

"OPTIONAL FEATURES
-----------------

256Kb Memory Module Kit (#0202)

This option increases the 5170 Model 68 memory from 256Kb to 512Kb. The kit consists of 18 modules that are plugged into existing
sockets on the system board. A maximum of one kit may be installed on Model 68 only. It is required on the Model 68 for installation of
#0209 and #0203."
