My system is using the 9301 Bootstrap/Terminator card and you have a 9312 card for yours, it almost looks to me that your 9312 is set to the wrong range of addresses and is coming up in a diagnostics...
With the settings set to 1.2mb, try booting to DOS and running ImageDisk in alignment mode. That might give you some additional clue. You should be able to manually step over tracks, and see what...
Those knobs don't do what you think. They are basically a directional pad. That being said you can dial in arbitrary aspect ratios through the OSD controls
