I am curious if anyone knew what particular models of System/360 this from would have been used for and the approximate date?

I have a partial pad of 10 of these 16 1/2" x 11" sheets (the pad originally had 25). I believe these were used by a college student who attended classes in the early 1970s and was later became a civil engineer / architect.

The earliest mention of these I've found so far are in this IBM document from December 1969:

Padded forms showing double word, word, halfword
and byte boundaries. Bytes marked to accommodate
packed-decimal digits. Provides for two records of 256
positions each, both in hexadecimal and decimal num-
bering. (25 per pad-11 x 16 1/2 )
This similar publication from IBM from 1973 also lists them, but their size is has shrunk to 6 1/2" x 11" and the number of sheets has grown from 25 to 50.

The actual forms I have were printed with the number "GX20-1711-1" & "UM/0251" in the top right.

This IBM publication does list the number with the extra "-1" but its dated 1980 and does fit the time period when the student was at college.
