Since very long I have had a DQ604 controller. In the eighties I ran it with an Ampex Pyxis 27 drive and it worked pretty well. Some time ago I picked up that card again and thought it could be worthwhile to get it running in a small system again. Possibly using the MFM emulator by David Gesswein.

The first problem was to find the manual. I know I once had it. Finally I found it and here is a scanned copy.

Then I recognised that obviously there were some kind of special formatter program needed for this controller. Where did I have put that one? Found a batch of RX02 disks that supposedly should have contained it. But they were not readable beyond track 43 and didn't seem to contain the file anyway. Bummer.

Then I found a bunch of old TU58 tapes. Luckily two seemed to have something that looked suspicious. I managed to image them after replacing tension band. They read well! The tapes are RT11 v4 tapes with the formatter on them.
One is SQ604C (RL01) and one is SQ604D (RL02).
