I've designed a copy of both the extender and receiver cards. Files are on GitHub. They're both confirmed to work in an actual 5161. Most of the parts can be found at Mouser with one exception: the 150ns delay line will have to be acquired elsewhere. But that's only on the receiver card, so if you only need the extender, you can get that all from Mouser, including the 62-pin D-sub cable.

The receiver card works in passive ISA backplanes, although you won't get the 14.318MHz clock signal which is required by some ISA cards.

    Enjoy!

    Excellent, and thanks!

Is there a good source for the monster cable that connects the two cards?

This is great! Is there template for ordering boards from any of the PCB fabricators?

Not really, I just used PCBWay. It's a bit more expensive since they are 4 layer boards with immersion gold edge fingers (30 degree bevel), but you could cheap out and get them with a HASL finish, or go to one of the really cut rate PCB houses. YMMV.
