This was a diskette that I got with my Compaq LTE 8086. It's a fairly uncommon disk, as it was something that you would send in after you filled it out (I made a copy of it of course).
Just for fun, I decided to fill it out (on my LTE/286), and when it go to the part that said "Please describe the problems you have experienced", I did exactly that!
