I dug this out of my big box o' ISA cards--it's an interesting beast. Based on the AMD LANCE chip, it has 4x10BaseT and one 10Base2 connection. I just tried this out on Win98 on a P1 system--it works fine. Includes disk and manual (you need the disk for configuration; after that Windows finds the card and installs drivers right from the standard distribution CD).
